The Basilica of San Giovanni in Laterano holds the official title of the Cathedral Church of Rome, it is the seat of the Bishop of Rome, which is the Pope's primary ecclesiastical title. As the mother church of all Catholic churches worldwide, it carries the formal inscription: 'Omnium urbis et orbis ecclesiarum mater et caput', 'Mother and Head of all Churches in the City and the World.'

Constantine the Great built the original basilica here in 313 AD on the grounds of the Lateran barracks of the imperial horse guard, immediately after signing the Edict of Milan granting Christianity legal status. This makes it the first large Christian basilica ever built. The building has been damaged and rebuilt multiple times; the current baroque facade was completed by Alessandro Galilei in 1735.

The complex also includes the Baptistery of San Giovanni (the oldest surviving baptistery in the Christian world) and the Sancta Sanctorum, the private papal chapel, accessible via the Scala Santa (Holy Staircase), 28 marble steps that tradition holds Jesus climbed in Pontius Pilate's palace, brought to Rome by St Helena. Pilgrims climb these stairs on their knees.
